Designing Future Dementia Care Infrastructure

Developing a structured dementia day program for rural and underserved communities.

Sweetgrass Care Systems focuses on environments, routines, and systems that support engagement, reduce strain, and help people remain at home longer.

Sweetgrass Care Systems LLC is currently establishing an initial community-based dementia day program in the Bismarck–Mandan region.

The program provides structured daytime support for people living with memory loss, while strengthening stability for their caregivers.

Core Principles

01
Environment-Centered Design
02

Spaces are structured to support clarity, safety, and participation for people living with memory loss, without relying on institutional or clinical settings.

Meaningful Engagement

Daily work supports attention, purpose, and continuity, rather than passive activity.

03
Caregiver Stability
04

Programs are structured to reduce strain and support long-term caregiving.

Rural Access

The model is built for rural and underserved communities, where access to specialized dementia support is limited.

In Development

Building a New Standard for Rural Dementia Care

Sweetgrass Care Systems is in the early stages of development, focused on:

  • Program design

  • Community partnerships

  • Alignment with regional health and support systems

Initial Impact

Reducing the physical and financial strain on families in rural North Dakota through accessible daytime support, while establishing a model for broader regional use.

Systems Integration

Working with existing regional health and support networks in ways that can be used across multiple communities.

Scope and Development Direction

Sweetgrass Care Systems is being developed as a regional approach to community-based dementia care.

Initial work is focused in the Bismarck–Mandan region to establish and refine a structured day program. This site is intended to test and develop the model, not define its full scope.

Over time, the aim is to extend this work across Western North Dakota, Montana, and Wyoming through a network of regionally anchored programs, with smaller sites serving surrounding rural communities.

This approach responds to gaps in access across geographically dispersed areas, while maintaining consistency in how the program is structured and delivered.

The model is intended for use in other rural regions facing similar constraints in access to dementia support.
